Dr. Shamoun believes that too many surgeons burn bridges by removing
far too much fat in the lower and upper eyelids. This may look good
for four to six months, and then suddenly one to two years later,
patients will often look older due to progressive skeletonization
of the face and eye regions. This can be seen in the following diagram,
which predicts the progressive and accelerated skeletonization often
seen with conventional lower eyelid fat removal procedures:
I will often preserve the fat in mots all patients, offering them
numerous alternatives that allow for rejuvenation for many years to
come. They include such procedures as:
1. Lower lid fat transposition (fat is rolled
down to fill soft tissue void below the bulge).
2. SOOF cheek mid fat lift (brings the cheek fat up and pushes the
lower lid fat posterior).
3. Transconjunctival fat removal with CO2 laser treatment.
4. Mid fat cheek lift with infraorbital rim implants.
5. Mid face cheek lift with canthopexy to tighten loose lower eyelid.
6. Upper eyelid skin removal with fat preservation and supratarsal
crease formation.
